Documents needed prior to start of services

Although it is not necessary, it would be very helpful if you downloaded a copy of the Consents and Psychosocial documents prior to our first session.

 

Completing these documents before the first session allows us to engage in the therapeutic services immediately.

If you haven't had the chance to send them to us prior to your first session, don't worry.  We can complete them together when you arrive.
